Mr. John is a 68 y/o male with a Hx of former smoker for the last 30-year, low socioeconomic class, anxiety disorder and risk-taking behavior.

He had developed a PVD with infected left foot, two admission in a period of 6 months with same complaining, non-adherence to stop smoking.

He was advised a BKA, but he could safe the leg after stop smoking and IV antibiotic therapy which he has a 50% of change to safe his leg.

He understands the susceptibility to developing a gangrene.

And also, he worried about the severity of complication that if he doesn’t stop smoking, he will lose his leg.

The physician explains to the patient and family the risk a developing of gangrene and lose his leg.

Advised to taking the IV antibiotic as prescribe and stop smoking.

Patient understand the needs to immediately stop smoking and adherence to drug regime that will prevent the complication and lose his leg.
