Stroke is a term used to describe the cessation of blood supply to an area of the brain due to ruptured blood vessels or thrombus in the blood vessels. This results in loss of brain function or diminished brain function in that area. It can either be a hemorrhagic stroke or an ischemic stroke. With a hemorrhagic stroke, the blood vessels rupture and disrupt the blood supply to the area of the brain affected. However, with ischemic stroke, there is a thrombus in the blood vessels that cuts the blood supply to areas of the brain affected leading to the death of brain cells (Grand Canyon University, 2018). The characteristics findings for stroke include facial drooping, slurred speech, weakness in one arm and/ or leg. The factors contributing to stroke include:

  1. Uncontrolled high blood pressure: this increases the pressure in the blood vessels, especially the blood vessels in the brain, and this could lead to rupture of the blood vessels, resulting in a stroke.
  2. High cholesterol: the high cholesterol in the blood vessels forms plaques that occlude the blood vessels in the brain causing ischemia and stroke.
  3. Diabetes: Diabetes increases the level of glucose in the bloodstream and this forms plaques in the blood vessels leading to occlusion and stroke.
  4. Smoking: smoking tends to form plaques in the blood which can result in a stroke.
  5. Atrial fibrillation: this can result in a buildup of blood clots in the blood vessels thus causing a stroke.Stroke (Cerebrovascular accident)

A stroke usually impacts the lives of patients and family, as they are unable to perform activities of daily living and other family responsibilities. Nurses have a role to play in helping patients and their family members by providing resources that include community resources, spiritual, and psychological resources to enable the patient and family to cope during the process. The nurse can also encourage patients and their family members to participate in rehabilitation sessions that may include speech therapy, physical therapy, and occupational therapy to improve patient’s compliance with the treatment plan. It is also important that the nurse encourage the patient to monitor predisposing factors to stroke. Common characteristic findings for a stroke include numbness or weakness in the face, arm, or leg, especially on one side of the body, confusion, trouble speaking, or difficulty understanding speech, trouble seeing in one or both eyes, trouble walking, dizziness, loss of balance, or lack of coordination, and severe headache with no known cause. The long-lasting effects of a stroke depends on rapid recognition and quick interventions. Stroke (Cerebrovascular accident) Quick intervention is needed with a stroke, preferably within 3 hours of the first symptoms. Acute care of a stroke includes stabilization and aggressive treatment if indicated. Once patient is stabilized then the patient will begin supportive services such as physical, occupational, and speech therapies to help with restoration of function to any areas of deficit. (Falkner & Green, 2018). When a patient has a stroke, it does not just affect the patient, it affects the whole family. A stroke is something that can leave a person severely disabled physically and cognitively depending on the severity. The patient may no longer be able to take care of themselves and may no longer be able to dependently feed, dress, or perform their activities of daily living alone, and may no longer be able to provide for their family. Stroke patients often develop depression and recovery from stroke requires physical, social, and emotional aspects of life. The nurse’s role in supporting patient’s psychological and emotional needs is to offer support by listening, using touch, being present, using silence when appropriate, offering encouragement and observing the patient for signs of distress. (Falkner & Green, 2018). An example of this is when a patient is unable to communicate then the nurse should do his/her best to try to comfort the patient, and try to address all of the patient’s possible needs and to just be there for him/her.

Abstract
This cross-sectional descriptive study evaluated registered nurses’ self-ratings of cultural competence on the
hematology/oncology unit at a large Northeastern urban children’s hospital. The Inventory for Assessing the Process
of Cultural Competence among Healthcare Professionals was used to measure 5 constructs of cultural competence.
The study findings show that there were significant correlations between the knowledge and skill subscales (ρ = .57,
P < .001) and the knowledge and desire subscales (ρ = .42, P < .05). The highest mean among the 5 subscales was
cultural desire (mean = 15.5), indicating that nurses were motivated to engage in the process of becoming culturally
competent. The lowest mean among the 5 subscales was cultural knowledge (mean = 11.2), followed by cultural skill
(mean = 11.8), indicating that nurses did not perceive themselves to be well informed in these areas. The findings from
this pilot study suggest that nurses on this pediatric oncology unit are most likely to possess cultural desire and cultural
awareness, but there is certainly opportunity to engage and educate the staff. Targeted interventions to improve
cultural competence on this inpatient unit are being explored and a larger scale study is being planned to assess the
cultural competence of nurses across the hospital.

According to the U.S. Census Bureau (2015), the minority population is projected to rise to 56% in 2060 compared with 38% in 2014. And as such, the current breadth
of ethnic and cultural backgrounds of individuals in the
United States requires that the nursing profession be culturally competent (Campinha-Bacote, 2002; Taylor &
Alfred, 2010). Culturally congruent care is considered a
standard of practice for nurses and a curriculum requirement for nursing schools (National League for Nursing
Accrediting Commission, 2016). Since nurses provide
the most direct health care services, the expectation to be
knowledgeable about diverse health beliefs and practices
is critical (Bauce, Kridli, & Fitzpatrick, 2014). Literature
supports the importance of practices, beliefs, value systems of diverse cultures, and emphasizes culturally congruent care (Hart & Mareno, 2013; Mareno & Hart, 2014;
Lin, Chang, Wang, & Huang, 2015) as this has shown to
improve patient outcomes and reduce health care disparities (Gallagher & Polanin, 2015; Garneau & Pepin, 2015).
Campinha-Bacote(2002) defined cultural competence as
an “ongoing process in which health care provider(s)
continuously strives to achieve the ability to effectively
work within the cultural context of the client (individual,

agreed-upon definition is lacking, the consensus is that cultural competence calls for a conscious process whereby providers, such as nurses, respect and appreciate values, beliefs,
and worldviews of diverse populations. To better serve
diverse populations, nurses need to be practicing culturally
congruent care. This is important as nurses’ perception of
themselves as culturally competent caregivers is reflected in
the way they communicate with patients and families.
A large urban children’s hospital serves a culturally
diverse patient population, and as a quaternary care center

This change is called the biggest health care system reform in the recent 50 years. There have been some pilot projects called “bundled care” on a small scale, which have brought up some promising outcome and positive feedback from patients (Crawley, 2019). Patients felt supported after discharge from the hospital. Each patient was looked after post-discharge; the readmission rate dropped; the overall time in the hospital was shortened; patients reported the improvement of their health (Crawley, 2019).

Health promotion means to increase people’s control over their health to improve their physical, mental and social well-being (Stemler et al., 2016). Appropriate technology means the appropriate health care model conforming to society’s social, economic, and cultural condition. Intersectoral collaboration refers to health care professionals from different disciplines work together to promote the public’s health and well-being (Stemler et al., 2016). This new model connects acute care in a hospital with community health care. Moreover, to the large extent, it emphasizes the importance of community health care, as the period of following up after discharge tends to be longer than initial treatment in hospital.
