Practice Concern in My Area of Practice

A practice concern in my workplace would be healthcare errors due to nurse and nursing assistant shortages. At my work, there are usually 2 nurses and 4 aides per floor for around 30 patients on each floor. For about a month now my work has been lucky to have 3 aides and 1 nurse per floor with a nursing supervisor that rotates between floors as needed. My facility has held a 5-star rating through the state and other surveyors in the past and I believe this number is at risk of decreasing due to this shortage in staffing. There have been more patient falls in the last 4 weeks than we have had in the past 7-8 months prior to having a staff shortage. Personally, I feel cares for patients are rushed which places patients at risk for many things such as falls.

Evidence Based Practices

Evidence based practice (EBP) research has been done regarding nursing shortage. A study supported by the Agency for Healthcare Research and Quality was performed using HIT which estimated that nursing schools need to graduate 25 percent more nurses each year. However, insufficient nursing faculty, inadequate financial resources, limited physical capacity, and insufficient clinical education sites mean that schools today cannot produce this number of nurses. (Nursing shortage, 2011). Information technology (IT) supports EBP by allowing health care practitioners to access and evaluate clinical evidence as they formulate their patient care strategies (Wells, 2006). IT and EBP both perform immense roles in healthcare.

Evidenced Based Practice

Health care is constantly changing and evolving and so is technology. Technology can be used in many ways in health care. With information technologies and data mining patient’s health care information can be used to improve quality care. “Recent advances in data mining techniques have resulted in wider applications in medical research, such as investigations into the causes of cardiovascular disease, lung cancer and cystic fibrosis” (Yi Chao, 2013).” Evidence based practice is research based practice that is the best practice for positive patient outcomes. Overall, practitioners want to prescribe the best treatment to their patients. One concern noted by many practitioners is the overuse of opioid pain medication. Opioid medications are constantly being prescribed when there is little to no confirmed effectiveness for their therapeutic use. Even though the list of side effects and adverse reactions continues to grow, opioids are the most frequently prescribed medication in the United States. (Miller & Gold, 2015) With health information technology and data mining we can see how many opioids are being written and what common diagnoses they are being written for. The Agency for Healthcare Research and Quality (ahrq.gov) uses research and evidence to create best practice standards. Using evidence based practice can make health care safer and improve quality overall. AHRQ also tracks data in order to improve performance and to evaluate the U.S. Health system.

Practice Concern

Low back pain is a common medical complaint in the emergency department. There are multiple treatment options for acute low back pain. Pharmacological treatments, non-pharmacological treatments, and injection therapies are available. The Agency for Healthcare Research and Quality (ahrq.gov) has done research review on the best treatment options for low back pain. Most trials were conducted in patients with non-radicular low back pain or mixed populations with primarily non-radicular low back pain. The findings were varied. For acute or subacute low back pain, NSAIDs, opioids and skeletal muscle relaxants were associated with small effects on pain versus placebo, and NSAIDs were associated with small effects on function. Acetaminophen and systemic corticosteroids were associated with no beneficial effects versus placebo. Overall, pharmacological therapies were associated with an increased risk of adverse events versus placebo. Non- pharmacological therapies including spinal manipulation, heat, massage, and low-level laser therapy were associated with some beneficial effects versus a sham therapy, no intervention, or usual care. Effects on pain and function were moderate for exercise, massage, and heat. Assessment and reporting of harms for non-pharmacological therapies were suboptimal but indicated no serious harms (ahrq.gov, 2016). Providers can use research such as this to educate patients on why opioids may not be the best treatment for acute low back pain.

Over the counter drugs and dietary supplements are
popular and widely used today. People use over the counter medications to help
treat things like the common flu, allergies, and pain. Over the counter
medications are available at pretty much any store. With these medications the
individual is responsible for following the instructions on the labels when it
comes to dosage and time frame in taking medications. One still must be
cautious when taking these medications, if not taken correctly or misused, it
can cause dangerous effects. Some examples of over the counter medications
include: aspirin, acetaminophen, ibuprofen, and naproxen. Some potential risks
of taking aspirin daily include “gastric bleeding, increased time for blood clotting,
and Reye’s syndrome” (Levinthal, 2016, p. 272). Ibuprofen can cause kidney damage,
or kidney failure.

Dietary supplements are anything from vitamins, herbs,
plants, minerals. In a national survey in 2007, approximately 18 percent of
American adults reported using “natural products” (dietary supplements other
than vitamins and minerals) in the past year” (Levinthal, 2016, p. 276). Many
people take these supplements to help with their moods, energy, or simply to
help make them feel better. Government regulations for these medications are
different from over the counter or prescription medications. These supplements
are not required to be tested for effectiveness or safety. Some effects of
taking dietary supplement can include: nausea, headache, upset stomach,
diarrhea, dizziness, etc.

Over-the-counter drugs the potential risk of using these types of over the counter drugs   Four out of five U.S. adults often take OTC meds for such complaints, as well as problems like skin disorders and digestive trouble.  For instance, acetaminophen, (Tylenol) which is the most over the counter medication used can be deadly as well as lead to strokes, heart attacks, anddeath.  It is stated, “Acetaminophen overdoses are also responsible for more than 150 deaths each year in the U.S”. (Dr. Mercola, 2013, p. 1) Diphenhydramine, an antihistamine, is another commonly  over the counter medication found in  sleeping pills  which can produce dizziness and drowsiness  as well as craving like illicit drugs heroin, cocaine etc. PPI  Over the counterproton pump inhibitors known as (PPI) used commonly for   heartburn and acid reflux , “can cause potentially serious side effects, including pneumonia, bone loss, hip fractures, heart attack, and infection Clostridium difficile” and gastral bacteria as well as craving,  tolerance and dependency on the PPI  being taken drug. Nonsteroidal anti-inflammatory ( NSAIDs), such asibuprofen, aspirin, naproxen is associated with  serious side effects such as cardiovascular problems, Gastral bleeding, hypertension, and kidney  problems,  as well as irregular or rapid heartbeat, which can increase the risk for stroke and heart attack

Dietary supplements can be classified as herbal and non- herbal with herbal being used more in society than noon herbal.  “Unlike OTC preparations, dietary supplements have not been evaluated by the FDA for safety and efficacy”  (Levinthal, 2016, p. 278)     Even though the scientific research is not finished on dietary supplements nine herbs, which have been researched Ephedra. Ginkgo, Ginseng, Hawthorn, Kava, St. John’s wort Saw palmetto, Turmeric and Valerian and all have been proven to put individuals  at risk  for having one or more of the following symptoms dis-orientation extreme euphoria, and hazardous irregular heart changes nausea, high blood pressure upset stomach, and liver damage.

Over-the-counter drugs are medications that people can buy without a prescription. These medications can include “pain, coughs and colds, diarrhea, constipation, acne and others” (NIDA, 2018, para. 1). Many people abuse OTC medicines and can become addicted to it. OTC drugs can have a negative impacts and cause “health problems including memory loss, kidney failure, heart problems and death” (Addiction Center, 2018). Dietary supplements are a common over the counter medicine that people take to lose weight. They are made up vitamins, minerals and herbs, they can cause harm when you take too much or can interact with your medications. Vitamin K decreases effectiveness of blood thinners, St. John’s Wort decreases certain drugs effectiveness, and some antioxidants decreased chemotherapy (NIH, 2018). Many dietary supplements are not FDA approved and don’t have a good quality. Taking too much of certain vitamins can cause health problems “too much vitamin A can cause headaches and liver damage, reduce bone strength, and cause birth defects. Excess iron causes nausea and vomiting and may damage the liver and other organs.” (NIH, 2018, para. 8).

Besides the satisfaction of personal accomplishment, athletes often pursue dreams of winning a medal for their country or securing a spot on a professional team. the use of performance-enhancing drugs has become increasingly common (Perrens, 2016).

Drug abuse in athletes is a significant problem that has many potential underlying causes. The drive to be the best in sport dates to ancient times, as does the use of performance-enhancing substances (Perrens, 2016). With the ever-mounting pressures faced by athletes, it is not surprising that drug abuse by athletes exists across essentially all sports and age groups.

According to the National Institute of Drug Abuse, some steroid user may become addicted to the drugs (DeLessio, 2016), there been evidence linked to continued use, despite the negative effect whether its social or physical problems.

Individuals who abuse steroids can experience withdrawal symptoms when they stop taking steroids, such as mood swings, fatigue, restlessness, loss of appetite, insomnia, reduced sex drive, and steroid cravings. The most hazardous of the withdrawal symptoms is depression, because it sometimes leads to suicide attempts. If left untreated, some depressive symptoms associated with anabolic steroid withdrawal have been known to persist for a year or more after the abuser stops taking the drugs (American Psychiatric Association, 2013).

The competitive nature of sports can entice athletes and coaches to seek performance-enhancing drugs. Coaches feel the pressure to have a winning team from schools and universities and athletes are groomed from the time they are young to focus on ultimate performance and victory. Careers are on the line if athletes and coaches cannot meet the winning standards that sports organizations place on them. Society’s view of sports and the large sum of money paid to athletes makes it difficult for individuals to rely solely on their own strength and athleticism. Athletes can also become obsessed with their bodies and strive for a perfect physic. This is especially true with weight lifters since the quality of their physic is what brings them victory. Some individuals suffer from muscle dysmorphia, which means that they have a distorted view of their bodies when they look in the mirror and only see imperfections and weakness (Levinthal, 2016). This type of obsession can cause an individual to become dependent on performance-enhancing drugs. The emotional and physical symptoms that occur when an athlete stops steroids can also motivate them to continue taking these drugs. In addition, many athletes are superstitious and may believe that they cannot perform well without the use of these drugs (Levinthal, 2016).

A Closer Look at Gulliver

Paragraph 1

In Jonathon Swift’s novel Gulliver’s Travels, Gulliver’s personality is shown through the language of Swift. In the novel, Gulliver comes across as a humorless and very factual and formal person. The book is written like a log — there are no opinions, only facts are included. He is the narrator for the story, offering no insight and never detecting any double meanings. Never digging for information, he tends to be rather naive and accept things at face value.

Paragraph 2

Throughout the novel, Swift does an excellent job of revealing what Gulliver is really like by putting him in a variety of situations and showing his reactions in those instances. He shows that Gulliver is ultimately a humorless, factual, and naive person. He does not show compassion or reflective thinking, but rather is akin to the narrator of the story. This story would be very different if it had a light-hearted and funny person as the main character. One that was cognizant of the events unfolding. Perhaps Jerry Seinfeld . . .

Paragraph 3

Naive is another thing that Gulliver is. Though he captures surface meaning with complete accuracy, he is not a deep thinker and the relevance of things escapes him. This book is a satire, poking fun at government and social issues. However, Gulliver does not recognize it as a satire. The audience does though. We see situations in the book which parallel our own lives. For example, the Lilliputians fought a war over which end their eggs should be cracked on. The Big Endians, who are considered religious rebels for their egg cracking beliefs, think all eggs should by cracked at the big end. The reason there is a conflict is that there exists a royal edict that declares that eggs should always be cracked at the small end. Lustrog, a religious prophet, has said that “all true believers shall break their eggs at the convenient end” (pg. 48). This war may seem to have a silly cause to the reader. After all, who really cares what end an egg is broken at? It is in this way that Swift causes the reader to question what most wars are about. Are not some of the causes of our modern day wars just as peculiar?

Paragraph 4

In the book, Gulliver is portrayed as a relatively humorless individual. Even when he is in a funny situation, his response is not laughter, but rather a straight faced account of events. For example, not long after Gulliver is first discovered by the Lilliputians, he wakes up to find that one of them has “put the sharp end of his half-pike a good way up into my left nostril, which tickled my nose like straw, and made me sneeze violently . . .”(pg. 17). He calls the incident “a very ridiculous accident,” as if it were just a childish prank.

Paragraph 5

Gulliver is also a very factual and formal person. In his accounts of his travels, his opinion is never really revealed. Gulliver’s factual and formal persona is shown well in the following quote:

It would not be proper for some reasons, to trouble the reader of our Adventures in those Seas: Let it suffice to inform him that our Passage from thence to the East-Indies we were driven by a violent storm to the Northwest of Van Diemen’s Land. By an Observation we found ourselves in the latitude of 30 degrees 2 minutes South. Twelve of our crew were dead by immoderate labour, and ill food; the rest were in very weak condition.

This passage shows that he gives very straightforward accounts of his events. Everything seems very precise, down to the latitude. He tells about the deaths of his shipmates in a very matter-of-fact way, not mentioning his personal feelings about the matter. He shows no compassion toward them and does not mention any names of friends or others that he might miss.

Often it is advantageous to have a friend proofread your paper for obvious errors. Handwritten corrections are preferable to uncorrected mistakes.

Use a standard 10 to 12 point (10 to 12 characters per inch) typeface. Smaller or compressed type and papers with small margins or single-spacing are hard to read. It is better to let your essay run over the recommended number of pages than to try to compress it into fewer pages.

Likewise, large type, large margins, large indentations, triple-spacing, increased leading (space between lines), increased kerning (space between letters), and any other such attempts at “padding” to increase the length of a paper are unacceptable, wasteful of trees, and will not fool your professor.

The paper must be neatly formatted, double-spaced with a one-inch margin on the top, bottom, and sides of each page. When submitting hard copy, be sure to use white paper and print out using dark ink. If it is hard to read your essay, it will also be hard to follow your argument.

Discussion Questions (DQ)

Initial responses to the DQ should address all components of the questions asked, include a minimum of one scholarly source, and be at least 250 words.
Successful responses are substantive (i.e., add something new to the discussion, engage others in the discussion, well-developed idea) and include at least one scholarly source.
One or two sentence responses, simple statements of agreement or “good post,” and responses that are off-topic will not count as substantive. Substantive responses should be at least 150 words.
I encourage you to incorporate the readings from the week (as applicable) into your responses.

Weekly Participation

Your initial responses to the mandatory DQ do not count toward participation and are graded separately.
In addition to the DQ responses, you must post at least one reply to peers (or me) on three separate days, for a total of three replies.
Participation posts do not require a scholarly source/citation (unless you cite someone else’s work).
Part of your weekly participation includes viewing the weekly announcement and attesting to watching it in the comments. These announcements are made to ensure you understand everything that is due during the week.

APA Format and Writing Quality

Familiarize yourself with APA format and practice using it correctly. It is used for most writing assignments for your degree. Visit the Writing Center in the Student Success Center, under the Resources tab in LoudCloud for APA paper templates, citation examples, tips, etc. Points will be deducted for poor use of APA format or absence of APA format (if required).
Cite all sources of information! When in doubt, cite the source. Paraphrasing also requires a citation.
I highly recommend using the APA Publication Manual, 6th edition.

Use of Direct Quotes

I discourage overutilization of direct quotes in DQs and assignments at the Masters’ level and deduct points accordingly.
As Masters’ level students, it is important that you be able to critically analyze and interpret information from journal articles and other resources. Simply restating someone else’s words does not demonstrate an understanding of the content or critical analysis of the content.
It is best to paraphrase content and cite your source.

LopesWrite Policy

For assignments that need to be submitted to LopesWrite, please be sure you have received your report and Similarity Index (SI) percentage BEFORE you do a “final submit” to me.
Once you have received your report, please review it. This report will show you grammatical, punctuation, and spelling errors that can easily be fixed. Take the extra few minutes to review instead of getting counted off for these mistakes.
Review your similarities. Did you forget to cite something? Did you not paraphrase well enough? Is your paper made up of someone else’s thoughts more than your own?

Late Policy

The university’s policy on late assignments is 10% penalty PER DAY LATE. This also applies to late DQ replies.
Please communicate with me if you anticipate having to submit an assignment late. I am happy to be flexible, with advance notice. We may be able to work out an extension based on extenuating circumstances.
If you do not communicate with me before submitting an assignment late, the GCU late policy will be in effect.
I do not accept assignments that are two or more weeks late unless we have worked out an extension.
As per policy, no assignments are accepted after the last day of class. Any assignment submitted after midnight on the last day of class will not be accepted for grading.
