Health information of patients does not only depend on the diagnoses, objective observations, and recreational activities but also the personal impressions about the clients, their characters, lifestyle, and other recreational activities (Pathak, 2019). Improper disclosure of patient information could potentially harm their reputation and lead to cases of personal humiliation, financial fraud, and loss in great opportunities.

Various people practice medicine today. They include physicians, surgeons, nurses, nurse practitioners, and other healthcare personnel. The large number of people visiting hospitals always leaves their information in the hospital’s records (McLeod, 2018). It is for this reason that the healthcare professionals need to protect this information by ensuring strict measures to ensure the confidentiality to patient information. Therefore, healthcare organizations do not condone careless behaviors such as sharing patient information in public, indiscretion, and maliciously using this information.

PICO (T) Question

The PICO (T) question identifies the research problems and the major components that constitute the research. In exploring the research about confidentiality in patient information, this research develops a PICOT question that will enable the researchers to drive the research process. The PICO question is: Patients between the ages of 45-70 years who have signed up for the hospital’s portal are likely to lose their private information for lack of understanding of the portal compared to patients between 16-40 years of age.

I identified six major strategies that can be applied to increase the effectiveness and rigor of the database search for the PICOT question. They include conceptualizing the search process, using relevant vocabulary, revising the search process, combining concepts and terms using Boolean operators, learning about the rules and regulations of database search, and searching different databases.

My muscles are weak

History of Present Illness

The patient is a female adult aged 38 years old. She was reported with a three-month history of arthritis after experiencing muscle weakness and mild headache. She started experiencing the symptoms in the past five-months months (May 2021). The patient at that time had hard joint pain in the knees, legs, and arms. After running the diagnosis, she was found to be suffering from Arthritis. To alleviate the symptoms, she was advised to take a short course of corticosteroids. Later on, the patient was kept on methotrexate, but the medication was not effective after a month, and she explained her concerns to the physician and was asked to stop taking the medication. The symptoms experienced by the client began worsening after two months when she stopped taking the methotrexate medication. She could vomit, experience nausea more often, sleepless nights, worsening headaches, and weak. She reported that loss of consciousness was experienced. Headaches were worse, making her not stand for about three to four hours, and occurred twice a week.

The patient reported that she had experienced an increase in the frequency of headaches that could last the whole day in the past month. With the increased headache, she reported that she could lie down, unable to stand. The patient is currently experiencing unbearable pain concurrently in the knee joints and arms, which leads to swollen and hot joints. She reported that she was experiencing dry mouth that made it difficult for her to Swallow food and felt a burning sensation in her eyes. During the night, she could experience excess sweating even if it was cold and mild fever, which made her unable to sleep comfortably and could stay awake most of the night.

She reported that lack of sleep during the night affected her day since she could mostly doze off while at work. While she was at some two days ago, the patient felt a constant and unbearable headache that left her to sit in one position for about two hours. Later on, she asked for permission and left for the day. No appetite to eat any food, and she tried to swallow. She could vomit after some hours. She visited the toilet normally, but once in a day, the stool and urine appeared to be expected. Due to lack of eating and vomiting after eating a small portion, her body was weaker; hence, she could not even take out of her bedroom without being supported. Her brother and a cousin helped the patient who arrived in the hospital after not eating for two consecutive days. While at the hospital, nurses discovered that patient had rashes on her face and extended to her nose and eyelids. Upon inquiring from the patient for how long she had the rashes, she was not aware of the rashes preset on her face.

The rashes were reported to increase each day the patient was in the hospital. Examination revealed she had lost 30 pounds of weight, and a deceases was seen in her muscle mass. Due to a lack of appetite, the patient experienced satiety and nausea. Her temperature was a bit high, and no loss of sensation was reported. Depression, mood, and frustration were reported since the patient could not carry on her daily routines or get time to socialize with her peers.

Past Medical History:

Hypertension, headache, and loss of appetite.

Gastric hernia and ulcers.

Ibuprofen 500 mg three times a day.

Allergies

Due to the patient experiencing ulcer symptoms, she was asked to avoid taking foods that contain acidity, such as red beans, and advised to take milk in more frequently. To remove the gastric hernia, she underwent a minor surgery that was eight years ago.

 

Social history

The patient completed her high and progressed to do her career course in Computer Science, she graduated with first-class honor. Upon her completion, she got a job opportunity at Microsoft and still working in the company. The patient currently lives with her cousin after the husband and children relocated to another state where the husband got green pasture and she is yet to join him. The patient career involves Information Technology mainly software development.

Family History

She is a married woman with three children, two girls, and one boy. She reported being taking minimal alcohol during functions. Her mother is alive and doing farming in their rural home while her dad died last year, 2020, due to the Covid 19 pandemic.

Review of systems

Her Skin showed some rushes on the face, but the rest of the skin was normal. The skin looked dry, and skin hair was intact, easy bruising, and the skin showed signs of discoloration. The hematopoietic showed she experienced fatigue, headaches most of the time, non-fainting, and tinnitus negative. From the face and head, she appeared to be ok since it was had not swollen. She could hear normally but had a few discharges. The eye site was normal. No sign of coughing, wheezing, and chest pain was experienced.

 

 

 

 

General Physical Examination

Physical findings showed that the patient blood pressure was 110/95, the temperature was 36.7C, Respiratory 25and oxygen saturation 92%. The patient was able to recall her memories and past events and was able to speak in a usual manner and appropriately.

Assessment

In summary, the 38-year-old female patient had a history of arthritis, and she didn’t have a family history on the same. She presented herself at the facilities after a five-month history of headache, joint pain in the knee and hand, weak and loss of appetite. She has had unbearable headaches, sweating at night, weaker, nausea, and a sleepless night in the last two months. In the past week, she reported having burning eyes and experienced dry mouth; the test result showed low hemoglobin of 10.9 and normal red blood cells. (Risso, 2021).

Plan

Problem 1 Systemic Lupus Erythematosus (SLE) could be a condition that the patient was suffering from. The SLE can account for low hemoglobin, but it doesn’t explain the patients’ burning eyes and dry mouth. The doctor’s rashes on the patient face were not something to worry about since there were only seen in the pre-auricular area during the examination. The plan for this problem is to keep close to how the patient’s kidney functions. Currently, no appropriate treatment for acute exacerbation has been found.

Problem 2 headache

Since the head scan showed no sign of abnormality in the patient, they can advise her to take Naprosyn 400mg for about two months. In case the medication is not effective, the doctor can suggest narcotic analgesia. The patient has a history of depression, a tricyclic antidepressant that works effectively in her case.

 

Problem 3 Siogrens syndrome assessment

` The “Sjogren’s syndrome assessment” in most cases is associated with arthritis. An average estimation of 20- 32% is witnessed in those that are suffering from arthritis. The results during a laboratory showed a Positive SS-A, the scan showed no sign of abnormality, the possibility of dry eyes and mouth was the availability of Sjogren’s syndrome or trigeminal neuralgia. Research shows that patients who show signs of SS have abnormal counts in the cell line, while cytopenia patients have more than one cell line. We could rule out anemia to be the leading cause of muscle weakness and headache. (Suzuki et al., 2020) The plan in this scenario was to help find out if the eye dryness was due to Schirmer. Medication that can be considered in this scenario could be hydroxychloroquine and immunosuppressant; this medication will help reduce joint pain. A developmental writing assignment means that there are multiple graded parts of the assignment. Breaking down such a large assignment into many component parts helps students by providing more instances of feedback throughout the semester. The six parts of the assignments are described below. A. First, you must propose your topic. The formal paper must be a focused healthcare ethics topic that can be adequately addressed in a double-spaced 6-8 page paper. It cannot be a topic that has already been approved for another student. Topics must undergo an approval process before you begin your work. Approval is done on a first-come, first-approved basis. Please submit your topic request via the link provided. To save time you may want to list several topic options. The deadline for getting your topic approved is the Friday of Week 3. Gaining approval before the deadline is worth 20 points.

 

Here are some tips for choosing a topic:

1. A topic must be focused enough to be adequately addressed in a 6-8 page which is double-spaced.

2. Here is an example of a topic that is much too broad to be adequately addressed in a paper of this length: The Ethical Considerations of Football Concussions. Entire books are written on this topic. A more focused topic is required. For example, a topic like The Impact of Baseline Concussion Testing on the Ethics of Youth Football Participation would be much more appropriate.

 

B. The second part of this assignment is completing the Formal Paper Quiz. This quiz is due on Friday of Week 4. The quiz will focus on the formal paper’s instructions. This quiz is worth 10 points.

 

C. The third part of the assignment is identifying the stakeholders and health care provider groups that will be presented in the third and fourth parts of the Formal Papers. Completing this part of the assignment is worth 50 points. This assignment is due on the Friday of Week 6.

 

D. The fourth part of the assignment is submitting an annotated bibliography consisting of at least ten peer-reviewed journal articles relevant to your approved topic. This assignment is due on the Monday of Week 9. Each article should have a properly constructed reference (either in APA or AMA format – pick one and use it throughout the assignment). Following each reference, you should write a 1-2 paragraph annotation that includes an original synopsis of the article’s purpose, conclusions, salient points, and relevance to the Formal Paper assignment in a paragraph. Regarding relevance to the Formal Paper assignment, connections should be drawn to important constructs of the assignment. These constructs include ethics, healthcare, autonomy, beneficence, non-malfeasance, justice, stakeholders, and healthcare professional groups. Original writing means that the annotation is paraphrased and written in your own words. This must be submitted as a Microsoft Word document. The annotated bibliography is worth 100 points.

 

F. The fifth part of the assignment is completing a peer-review of two other student’s papers. Completing the peer reviews is worth 50 points. A peer-review draft (approved topics only) of your paper is due no later than Monday of Week 10. Your reviews of the other student’s papers are due by Monday of Week 11. This should not be a rough draft; rather the expectation is that you have edited this paper heavily and refined it with the assistance of the ISU Writing Center.

 

G. The sixth part of this assignment is submitting a final copy of your formal paper (approved topics only). The deadline for submitting this assignment is the Friday of Week 14. The final copy is worth 120 points.

Reminder about Peer Reviewed Journal Articles

Peer-reviewed articles (PRJA) are a subset of scholarly articles. They are special because they are published in academic journals that explicitly state that submitted manuscripts will undergo peer-review.

In academic publishing, the goal of peer review is to assess the quality of articles submitted for publication in a scholarly journal. Before an article is deemed appropriate to be published in a peer-reviewed journal, it must undergo the following process:

· The author of the article must submit it to the journal editor who forwards the article to experts in the field. Because the reviewers specialize in the same scholarly area as the author, they are considered the author’s peers (hence “peer review”).

· These impartial reviewers are charged with carefully evaluating the quality of the submitted manuscript.

· The peer reviewers check the manuscript for accuracy and assess the validity of the research methodology and procedures.

· If appropriate, they suggest revisions. If they find the article lacking in scholarly validity and rigor, they reject it.

· Because a peer-reviewed journal will not publish articles that fail to meet the standards established for a given discipline, peer-reviewed articles that are accepted for publication exemplify the best research practices in a field. PRJAs do not come from magazines or newspapers. Magazine and newspaper articles sometimes identify peer-reviewed journal articles, so it is possible to use them in order to find a peer-reviewed journal article. A published journal is the entity that defines whether or not its articles are peer-reviewed. Another word that is a synonym of peer-review is refereed. Once you find an article that appears to support your response, you should check with the journal to determine if it is peer-reviewed or not. Virtually all journals will have a website.

· Let’s say that you have articles from 3 periodicals that appear to support your response. They are The Journal of the American Medical Association, Nature, and The New York Times. Take a moment and review these websites, remembering that if a journal does not state that it is peer-reviewed, it is not.

You must proofread your paper. But do not strictly rely on your computer’s spell-checker and grammar-checker; failure to do so indicates a lack of effort on your part and you can expect your grade to suffer accordingly. Papers with numerous misspelled words and grammatical mistakes will be penalized. Read over your paper – in silence and then aloud – before handing it in and make corrections as necessary. Often it is advantageous to have a friend proofread your paper for obvious errors. Handwritten corrections are preferable to uncorrected mistakes.

Use a standard 10 to 12 point (10 to 12 characters per inch) typeface. Smaller or compressed type and papers with small margins or single-spacing are hard to read. It is better to let your essay run over the recommended number of pages than to try to compress it into fewer pages.

Likewise, large type, large margins, large indentations, triple-spacing, increased leading (space between lines), increased kerning (space between letters), and any other such attempts at “padding” to increase the length of a paper are unacceptable, wasteful of trees, and will not fool your professor.

The paper must be neatly formatted, double-spaced with a one-inch margin on the top, bottom, and sides of each page. When submitting hard copy, be sure to use white paper and print out using dark ink. If it is hard to read your essay, it will also be hard to follow your argument.

Discussion Questions (DQ)

Initial responses to the DQ should address all components of the questions asked, include a minimum of one scholarly source, and be at least 250 words.
Successful responses are substantive (i.e., add something new to the discussion, engage others in the discussion, well-developed idea) and include at least one scholarly source.
One or two sentence responses, simple statements of agreement or “good post,” and responses that are off-topic will not count as substantive. Substantive responses should be at least 150 words.
I encourage you to incorporate the readings from the week (as applicable) into your responses.

Weekly Participation

Your initial responses to the mandatory DQ do not count toward participation and are graded separately.
In addition to the DQ responses, you must post at least one reply to peers (or me) on three separate days, for a total of three replies.
Participation posts do not require a scholarly source/citation (unless you cite someone else’s work).
Part of your weekly participation includes viewing the weekly announcement and attesting to watching it in the comments. These announcements are made to ensure you understand everything that is due during the week.

APA Format and Writing Quality

Familiarize yourself with APA format and practice using it correctly. It is used for most writing assignments for your degree. Visit the Writing Center in the Student Success Center, under the Resources tab in LoudCloud for APA paper templates, citation examples, tips, etc. Points will be deducted for poor use of APA format or absence of APA format (if required).
Cite all sources of information! When in doubt, cite the source. Paraphrasing also requires a citation.
I highly recommend using the APA Publication Manual, 6th edition.

Use of Direct Quotes

I discourage overutilization of direct quotes in DQs and assignments at the Masters’ level and deduct points accordingly.
As Masters’ level students, it is important that you be able to critically analyze and interpret information from journal articles and other resources. Simply restating someone else’s words does not demonstrate an understanding of the content or critical analysis of the content.
It is best to paraphrase content and cite your source.

Late Policy

The university’s policy on late assignments is 10% penalty PER DAY LATE. This also applies to late DQ replies.
Please communicate with me if you anticipate having to submit an assignment late. I am happy to be flexible, with advance notice. We may be able to work out an extension based on extenuating circumstances.
If you do not communicate with me before submitting an assignment late, the GCU late policy will be in effect.
I do not accept assignments that are two or more weeks late unless we have worked out an extension.
As per policy, no assignments are accepted after the last day of class. Any assignment submitted after midnight on the last day of class will not be accepted for grading.

Communication
